2. Ibogaine Carries Serious Safety Risks

3. Cardiac & Medical Risks

Cardiac complications are the leading cause of ibogaine-associated deaths. A baseline cardiac evaluation — including 12-lead ECG — is essential before any ibogaine administration.

Known medical risks associated with ibogaine include, but are not limited to:

  • Cardiac arrhythmias — ibogaine prolongs the QT interval, which can cause potentially fatal ventricular arrhythmias (including Torsades de Pointes and ventricular fibrillation).
  • Bradycardia — significant slowing of heart rate during the treatment session.
  • Seizures — particularly in individuals with a history of seizure disorders.
  • Acute respiratory failure — risk is elevated in individuals with underlying respiratory conditions or those who have recently used opioids.
  • Prolonged and intense psychedelic experience — typically lasting 12–36 hours, which can be psychologically destabilizing.
  • Ataxia and coordination impairment — lasting for the duration of the session, creating fall risk.
  • Nausea and vomiting — common during the session, with aspiration risk.
  • Drug interactions — ibogaine interacts with a wide range of medications (see Section 4).

4. Contraindications & Drug Interactions

Ibogaine is contraindicated — meaning it may be medically unsafe — in individuals with, among other conditions:

Critical Medication Interactions

Ibogaine interacts dangerously with many commonly prescribed medications. Known high-risk interactions include:

  • SSRIs and SNRIs (antidepressants such as fluoxetine, sertraline, venlafaxine) — risk of serotonin syndrome; medications must typically be tapered and discontinued under medical supervision well in advance.
  • MAOIs — severe and potentially fatal interaction.
  • Opioids — ibogaine is sometimes used in opioid treatment; however, opioid levels must be carefully managed as both substances affect respiratory and cardiac function.
  • QT-prolonging medications — including many antibiotics, antifungals, antipsychotics, and antihistamines.
  • Lithium — increased seizure and cardiac risk.
  • Stimulants — cocaine, amphetamines, and ADHD medications increase cardiac risk.
  • Methadone — methadone itself prolongs QT; combination requires extreme caution.

Ibogaine is classified as a Schedule I controlled substance under the Controlled Substances Act, meaning it is illegal to possess, distribute, or use it in the United States without DEA authorization (which is rarely granted except for narrow research purposes). It is not FDA-approved for any medical indication.
